The Official Hasha International Karate Licence

This is not a Personal Injury Policy. The insurance provides 3rd party cover in the event that should you damage property or injure another student during training. It will not pay benefits should you be off work for a length of time as a result of an injury sustained during training.

I the trainee indemnify HASHA INTERNATIONAL KARATE in respect of any injury caused to another member or against losses caused to the club by any act of mine or default.
I the trainee understand that the training and practise of Shotokan Karate or any Martial Art can be dangerous and I agree to personally bear all losses caused by injury whilst engaged in it. Renewal of the annual licence is the responsibility of the individual member and that out of date licences "WILL BE BACK DATED". Licences are valid for one year. HASHA INTERNATIONAL KARATE reserve the right to decline an application without stating a reason.
